Can I save the malnourished chi puppy I obtained today?

I received a chi puppy today, she maybe 2 mths old, she had been taken from the mother at 4-5 wks old. I had nothing to do with this I am trying to save this baby. Anyway she's extremely malnourished, which causes her to have tremors and difficulty walking. She gets weak easily. I think she may be hypoglycemic. I have been feeding her canned pedigree puppy food every couple hours and making sure she has plenty of water. I plan on worming her tomorrow. I'm looking for any advice on what I can do for her. She is small and pitiful i'm hoping with the right nutrition she'll pull through and get stronger.
